A 1-1.5 hour meeting to discuss your unique situation and individual needs. We will talk about your end of life wishes and how I can best help support you and your family during this time.
Death and Vigil planning is when we will create a formal plan to match your wishes for your end of life.
A one hour meeting when we will discuss your very specific wishes for your end of life. I will ask you to think about what you want your death to look like - who do you want with you, where do you want to be, what do you want to smell and hear, what kind of comfort measures do you want taken, etc? We will create a formal, written plan that you can share with your care team so they know your wishes and what their role will be in your final days, hours, and moments.
This includes two 1 hour meetings where we discuss your end of life wishes, complete the appropriate documents, and finalize your plans. This can include designating a medical proxy, outlining wishes for end of life medical support (life saving measures, medications, mechanical life support, etc), and more.
